Turk loses a testicle but doesn’t tell JD. He overcompensates by wrestling people for the remote control. Dr Cox taunts JD for being a bad male role model for Sam. JD fights Turk and accidentally wins. He hides from Turk who wants revenge. Meanwhile Elliot overhears Dr Kelso saying he will be forced to retire. She also has a male patient with breast cancer who is embarrassed and doesn’t want anyone to know. Through that experience she realises that Dr Kelso really wants her help. The Janitor starts a newspaper called The Janitorial and when Dr Cox calls him stupid he prints a fake interview to irritate him.

Carla and Elliot try to save Dr Kelso’s job. Jordan advises them to get all the hospital’s important personnel to approach the board. But no one will help because they are scared of Dr Cox who wants Kelso to leave. Dr Cox is struggling to diagnose a patient and is annoyed when JD and Turk find the answer. Dr Kelso reminisces all day to an intern called Boone. Dr Cox realises that the enemy he knows is better than then one he doesn’t and they manage to save Kelso’s job. But in the end he leaves anyway, happy to go out on his own terms.

Elliot is trying to track down a patient who may be entitled to money from a financial settlement. JD offers to help her if they can pick up Sam on the way home. They play a prank at a fast food outlet which goes wrong but eventually it brings them closer together. Carla wants to have another baby but Turk isn’t sure. The Janitor encourages Ted to be more assertive but it leads to conflict. In Dr Kelso’s absence Dr Cox is in charge and he has to deal with the various problems.

After a bad day, Dr Cox decides to tell Jack about it but in the style of a fairy tale. The real story is that Elliott and JD struggle to diagnose a girl who is suffering badly. Dr Kelso announces that any doctor’s working beyond their shift will be sent home because otherwise the hospital could be liable for their mistakes. JD and Elliott discuss their almost kiss but agree it meant nothing.
